**STRIVE MENTORSHIP
SERVICE AGREEMENT**
This Service Agreement (“Agreement”) is entered into between Strive Mentorship (“Service
Provider”) and Meagan Harris (“Client”) as of January 21, 2025.

1. SERVICES PROVIDED
Strive Mentorship agrees to provide the following services:
A. Business Coaching & Mentorship
● Non-medical home care business coaching and mentorship
● Startup guidance and operational support
● Licensing and compliance mentorship
● Business strategy and implementation support
● All services included in the Strive Care flyer

B. Policies & Procedures Development
● Customized non-medical home care policies and procedures
● Documents aligned with industry standards
● Final delivery upon full payment

2. SERVICE FEES
The total cost for services under this Agreement is $950.00, broken down as follows:
● Business Coaching & Mentorship: $350.00

● Policies & Procedures: $600.00

3. PAYMENT TERMS
The Client agrees to the following payment schedule:
● Initial Payment: $475.00
○ Due: January 21, 2025
● Remaining Balance: $475.00
○ Paid in three (3) installments of $158.33 each, due on:
■ February 21, 2025
■ March 21, 2025
■ April 23, 2025

4. RELEASE OF POLICIES & PROCEDURES
⚠️ Important Policy Notice
● Policies and procedures will only be released once the full balance of $950.00 is paid in
full
● Partial payments do not authorize document release
● No exceptions to this policy

5. NO REFUNDS
All payments made under this Agreement are non-refundable, including:

● Initial payment
● Installments
● Mentorship services
● Policy and procedure preparation

6. CLIENT RESPONSIBILITIES
The Client agrees to:
● Provide accurate and complete information
● Attend and participate in mentorship sessions
● Submit payments by stated due dates

Failure to comply with payment terms may result in service suspension.

7. INDEPENDENT SERVICE DISCLAIMER
Strive Mentorship provides educational and mentorship services only and does not guarantee:
● Business licensing approval
● Regulatory acceptance
● Revenue or business success

Client remains responsible for all business decisions and compliance.

8. AGREEMENT ACCEPTANCE
By signing below, the Client confirms understanding and acceptance of all terms.
