To request CLE credit, please enter the name/date of the program below along with other pertinent information and return this form at program’s end. We will then seek approval from the appropriate administrative entity.

Please note: this form is not intended for use with the Bench & Bar or Global Series programs.

CLE is available only for FCBA members and paid non-member registrants.
CLE not available for Puerto Rico.
