I am not under the influence of any chemical substance, including alcohol. I understand that any physical activity involves a certain risk of injury. I understand that my participation in the Camp Bountiful “ROPE CHALLENGE COURSE” program is entirely voluntary.  

I understand that participation in the “ROPES CHALLENGE COURSE” activity offered through the Church of Jesus Christ of Latter-day Saints @ Camp Bountiful, involves a certain degree of risk that could result in injury.

In consideration of the benefits to be derived and after carefully considering the risk involved and in view of the fact that the Church of Jesus Christ of Latter-day Saints is an organization in which membership is voluntary, and having full confidence that precautions will be taken to ensure the safety and well-being of myself, including having the ROPE Director or Instructors check and adjust any harnesses or helmets to ensure correct fit, safety and security while wearing the harness, I have carefully considered the risk involved and have given myself consent to participate in the “ROPE CHALLENGE COURSE” activity. 

                                                                                                   

In case of emergency, I hereby give my permission to the physician selected by the adult leader in charge to secure proper treatment, including hospitalization, anesthesia, surgery, or injections of medications as is deemed necessary.
