    Priorities assessment survey
  • The purpose of this survey is to gather valuable insights from community members to help inform the planning and implementation of the Title V Maternal and Child Health (MCH) program in Calaveras County. Your input will assist in identifying the most pressing health needs and priorities for women, mothers, infants, children, adolescents, and children and youth with special health care needs (CYSHCN) in our community.

  • Women's and Maternal Health

    Maternal health refers to the health of women during pregnancy, childbirth, and the postnatal period.

  • Perinatal and Infant Health

    Perinatal and infant health is the health of women and babies before, during, and after birth.

  • Child Health

    Child health refers to the health and development of children, including physical, mental, behavioral, and emotional health.

  • Adolescent Health

    Adolescent health refers to the changing transitions within physical, social, emotional, cognitive, and intellectual health of youth.

  • Children and Youth with Special Health Care Needs (CYSHCN)

    CYSHCN include infants, children, and youth from birth to age 21 who have one or more chronic physical, developmental, behavioral, or emotional conditions, and require additional health and support services, beyond those needed by children generally, as a result of their condition(s).
