Participation Waiver and Assumption Agreement
I, confirm that I am the above named Minor Child's parent and/or legal guardian.I permit Minor Child to participate in the Event, which includes all of the Event’s related and associated activities. I enter into this agreement freely and as a condition of receiving the opportunity for Minor Child to participate in the Event.
I remain legally responsible for any actions taken by Minor Child. I, on my own behalf and on Minor Child’s behalf, including both of our heirs, successors, and assigns, hereby release and agree to indemnify, hold harmless, and defend Central Denver Classical High School d/b/a St. John Paul the Great Catholic High School (“JPG”), its directors, officers, employees, agents, chaperones, coaches, owners/lessors of property used for the Event, or representatives associated with the Event, from any claim arising from or in connection with Minor Child’s participation in the Event, or in connection with any illness or injury (including death), cost of medical treatment in connection therewith, or any claim that may arise from any medical treatment Minor Child receives as a result of an injury or illness, including emergency medical treatment that JPG may request. I assume all risks incumbent with the Event on behalf of Minor Child and agree that I am responsible for all expenses associated with any injury Minor Child may incur in connection with the Event. I understand and agree that the Event includes athletic activities, and risks that I assume specifically include (without limitation) risks incumbent with athletic activities such as fractures, paralysis, concussion, or death. I agree to compensate JPG, its officers, directors, agents, employees, chaperones, or representatives associated with the activities for reasonable attorney's fees and expenses which they may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the gross negligence or willful misconduct of such person or JPG.
