By signing this document I consent to being photographed, filmed and recorded in any medium, by any staff of Worklinks Qld Ltd or their agents, including but not limited to any photographer, interviewer, creative agency or media organisation; for the purposes of promoting Worklinks Qld Ltd and it’s services.

I consent to and authorise the use and disclosure by Worklinks Qld Ltd and its Agents of any photographs, footage, words, images, quotes or other comments made by or attributed to me, and any other personal information about me, including sensitive information*, collected for the purpose of promoting and marketing Worklinks Qld Ltd.

I consent to and authorise the Material being made publicly available including by publishing it as or as part of any book, newspaper advertisement or article, television advertisement or programme, radio advertisement or programme and including on the internet (which by its nature may involve disclosure to overseas recipients in any country) and any other media at any time. I understand that once the Material is published that it will be in the public domain and that the Worklinks Qld Ltd and its Agents are limited in their ability to prevent subsequent use and dissemination of that Material even if information about me in it has ceased to be true or is otherwise misleading at the time of its use.

To the extent (if any) I own intellectual property rights in the Material or any part of the Material, I licence the Worklinks Qld Ltd and its Agents to copy and disseminate it in accordance with this consent.

I agree that Worklinks Qld Ltd and its Agents are not obliged to, and have not represented they will, publish the Material; do not have to identify me as author of any of the Material which they do publish or otherwise communicate; and may edit the Material prior to publication as they sees fit without first checking with me.

I understand that by signing this consent form I am agreeing to do so on the basis that there is no fee payable to me for such participation and that a representative of Worklinks Qld Ltd have explained the contents of this document to me.

I understand that the consequence of not providing consent is that I will not appear in the Material and that I retain the right to withdraw the consent granted in writing at any time prior to publication of the Material.

Collection of sensitive information  

Sensitive information, as defined in the Privacy Act 1988 (Cth) (Privacy Act), includes information about an individual’s health, racial or ethnic origin, political opinions, membership of a political association, religious beliefs or affiliations, philosophical beliefs, membership of any professional or trade association, membership of a trade union, sexual orientation or practices, criminal record, genetic information that is not otherwise health information, or biometric information. Subject to a few exceptions, the Privacy Act precludes an entity subject to the Australian Privacy Principles (APPs) (such as Worklinks Qld Ltd) from collecting sensitive information about an individual without that individual’s consent. If Worklinks Qld Ltd and its Agents collects sensitive information about an individual for the purpose of promoting and marketing Worklinks Qld Ltd and it’s services, Worklinks Qld Ltd and its Agents will only use that sensitive information for those purposes, and only to the extent that the sensitive information is directly relevant to those purposes.

Management of personal information by Worklinks Qld Ltd

Worklinks Qld Ltd manages personal and sensitive information it collects in accordance with the Privacy Act. Worklinks Qld Ltd collects your personal information for the purpose of obtaining your consent to the use and publication of your name and photographs, footage, words, images, quotes or other comments made by or attributable to you. Your personal information will not be used for any other purpose or disclosed to any other person or entity unless such a use and disclosure is permitted pursuant to the Privacy Act. If your personal information is not provided Worklinks Qld Ltd will be unable to confirm your consent to use or disclose the Material.
