    If you are traveling with children who have a different last name:

    In addition to the travel documents that all guests need to sail, you'll need a birth, marriage, divorce, or adoption certificate. Here are some examples of valid supporting documents:

    State, county or province-issued marriage license
    Divorce decree
    Government-issued name change document
    Official adoption paper

     

    If you are traveling with a minor child who is not your own:

    Adults who are not the parent and/or legal guardian of any minor child traveling with them are required to present the child’s valid travel documents and an original notarized letter signed by the child’s parents(s)/legal guardian(s). The required notarized letter from the child’s parent/legal guardian must authorize the traveling adult to take the child on the cruise, taking full responsibility for that child and give medical authority to the traveling adult. Please note, this form must be signed in the presence of a notary by at least one of the child's parents or legal guardians. You’ll need to provide a copy of this signed, notarized form to be presented at check-in. If you do not have a notarized letter at embarkation the child can be denied boarding. Penalties will apply.

     

    Please check with the cruise you are traveling with for specific forms required for minors not your own.
