Accommodation Application - 2025

In order to receive accommodation services from the Leukaemia Foundation, you must be eligible for, and registered with your relevant state patient travel and accommodation scheme, and be accompanied by a carer aged 18 or over.     Please note that 2 business days notice is required. If your request is for a date within 2 business days, or if you have any questions, please contact us directly at accommodation@leukaemia.org.au.

    Christmas Closure Information - Please Read

    Leukaemia Foundation Offices will be closed from 3.00pm on Tuesday, 24 December 2024 and will re-open again on Thursday, 2 January 2025.

    The last day for accommodation requests is Monday, 23 December 2024.

    Any accommodation requests received after this date will be reviewed by our team but may not be processed until after our return in 2025.

    Changes to this form, please read.

    We have recently made some changes to this online accommodation application form, these changes have been made in line with amendments to our assessment process, as well as some changes to the way we offer accommodation support.

    If you have completed this form before, you may notice some questions have been removed or changed, to allow for faster completion of this form and ease of use for the applicant. Some of these questions have been removed to allow further assessment to be done over the phone by our team.

    Blood cancer leaves people very vulnerable to infection and viruses, including COVID-19, and the Leukaemia Foundation has long prioritised heightened infection control, even before this pandemic. As we all learn to live with COVID-19, the Leukaemia Foundation has introduced a raft of new safety measures to further protect people with blood cancer, including those staying in our accommodation centres. COVID-19 vaccines give us the best shot at stopping the virus endangering the lives of the most vulnerable in our community. They’re excellent at slowing the spread of the virus in the general population, which is vital because we know the vaccines offer less protection to many of those with blood cancer, who remain vulnerable. As part of our new safety measures, anyone 16 years and over who stays in or visits our accommodation centres, including patients and their carers and families, must show they’re fully vaccinated, unless medically exempt or undergoing emergency treatment. We’re asking all our accommodation staff and contractors to be fully vaccinated, too. We’d like to thank everyone living and working in our accommodation already vaccinated for your help safeguarding the lives of people with blood cancer.
