MorDance After-School Class Waiver Form

Welcome to MorDance After-School Programs! We are thrilled that you have chosen to dance with us!

MorDance after-school is a dance program developed by MorDance, Yonkers’s premier Professional contemporary ballet. This ballet class offers structured creative learning to teach students the basic vocabulary of ballet, while fostering personal growth skills like self-confidence, focus, excellence, and determination. Dancing is a joyful, fun, and low-stress way to stay fit, feel strong, expressive, and healthy! This is a FREE class series that will continue through the end of the school year!

Waiver of Liability / Acknowledgment of Risk / Photo & Video Release

Please read carefully and sign below in order to participate in MorDance after-school.

I understand and agree that participating in any MorDance after-school class, or workshop, is a possibility of physical injury. I agree to release and hold harmless MorBallet INC (DBA MorDance), including its teachers, dancers, staff members, from any cause of action, claims, or demands now and in the future. I will not hold MorBallet INC (DBA MorDance) liable for any personal injury or any personal property damage, which may occur on the premises before, during, or after classes. Furthermore, I understand that I should be aware of my and/or my child’s physical limitations and agree not to exceed them. I am signing this waiver for myself and/or my child, I certify that I am over 18 years of age or the parent or legal guardian and have the right to waive these rights. Further, I authorize and agree that MorBallet INC (DBA MorDance) may record the class instructors ONLY for purposes of record-keeping, marketing, and instructor/content assessment and understand that MorBallet INC (DBA MorDance) will never record my child’s image.
