    The word "event' is used to describe any event or activity support the 150th. It does not mean that it has to be a "party" ex: a video series could be considered an event for the purposes of this form.

    proper consideration and maximum success of the proposed event.
    1. Please complete & submit as much detail as possible on this form. Emailing subsequent data could result in lost information.
    2. Submissions will be reviewed by the SIG 5 group and recommended based on this form. 
    3. If the University moves forward with your idea you will be contacted with details and possible requests to update/modify budget, timeline, etc.
