I, {name}, as the legal parent/guardian of {studentName}, I give my permission for them to enter the Cape Assist and Lower Township Healthy Youth Coalition’s (LTHYC) T-Shirt Design Contest. I confirm that the artwork/design submitted by my child is their own original work, free of any copyright, brands, or trademark infringements. I have read and agree to the contest rules and eligibility requirements as outlined on the contest page at www.lthyc.org/contest. I affirm that all information provided on the contest entry form is true and understand that any false information may lead to disqualification from the contest.
I agree that my child’s name, photograph, age, grade, school, likeness, voice, video, and artwork can be used by Cape Assist and LTHYC for promotional, advertising, and educational purposes in various formats, including print, digital, and online, indefinitely.
I also consent to Cape Assist and LTHYC making any changes they deem necessary to the submitted materials and waive my right to approve the finished product. Furthermore, both the student and I agree to protect Cape Assist and LTHYC, along with their officers, employees, and agents, from any legal action or costs arising from the production or sharing of the contest materials.
I understand that if my child’s entry is chosen, the only reward we will receive is the contest’s specified prize(s), without any additional compensation.
Should I choose to withdraw this consent, I understand that the withdrawal must be made in writing and sent via certified mail to Cape Assist, 3819 New Jersey Avenue, Wildwood, NJ 08260. Cape Assist has up to 60 days to respond to the consent withdrawal from the date they receive it.
