Thank you for your willingness to volunteer at one or more of Pueblo Spay & Neuter Now’s spay/neuter clinics. Your support is crucial in our mission to combat pet overpopulation and homelessness in Southern Colorado! Before you begin, we kindly ask you to read and sign this waiver and release of liability. Minors under the age of 18 must have a parents signature, we cannot accept volunteers under the age of 16.

Release of Liability:
In consideration of being permitted to volunteer, you release Pueblo Spay and Neuter Now, its directors, employees, and agents from any claims, demands, and causes of action arising from your participation in volunteer activities. This includes any claims related to personal injuries or damage to property that may arise, whether caused by negligence or otherwise.

Medical Treatment:
Should you require medical treatment while volunteering, this waiver includes a release of Pueblo Spay and Neuter Now from any claims related to such treatment.

Photographic Release:
If you selected "Yes" to the Photgraphic Release question above, you grant Pueblo Spay and Neuter Now the right to photograph or video record your participation in volunteer activities and to use these images for promotional, educational, or fundraising purposes.

This document is designed to be as broad and inclusive as permitted by Colorado law. If any portion is held invalid, the balance shall continue in full legal force and effect.
