Informed Consent and Acknowledgement
I hereby give my approval for my child’s participation in any and all activities prepared by Adventure Pirate LTD.
I am aware that the activities my child will be undertaking are physical and accidental injury/illnesses are possible. These may include, but are not limited to the risk of cuts, bangs, bruises, fractures, paralysis, or death.
If my child is injured, I agree to instructors administering first aid on my child. I am happy to be contacted if this happens.
I agree to any photos/videos of my child participating in activities to be used to promote Adventure Pirate LTD.
I am paying a termly, not weekly fee. If my child joins part way through a term AP will send me a pro rota invoice for the rest of the term which mut be paid upfront.
If my child misses a session/sessions due to illness or other commitments, the missed session(s) will not be rolled over to the next term and I will not be refunded.
Adventure Pirate's priority is the safety of children in their care. If Adventure Pirate staff feel that it is too dangerous to run a session due to extreme weather conditions, they will cancel the session. The cancelled session will not be rolled over to the next term and I will not be refunded for the cancelled session. Sessions will only be cancelled in extreme circumastances - yellow wind warnings and/or amber flood warnings.
HomeEd club sessions consist of 1hr 30mins of instructor led activities, followed by 30 minutes of optional free play time. I will sign my child over to staff at the start of the session and then sign them back out on completion of the session. I am responsible for my child before and after sessions, this includes during the optional free play time.
My child is not allowed to physically or verbally abuse any other children, instructors or any other adult at Adventure Pirate. If my child does physically or verbally abuse anyone at Adventure Pirate they will be asked to leave and may not be invited back to future sessions.
Adventure Pirate try to promote independance during HomeEd sessions. I am allowed to drop of my child and leave them in the care of Adevnture Pirate staff. I must be back promptly for the end of the session/start of free play time to ensure I supervise my child.
If I remain onsite, I must be out of site/distant from the group so that my child doesn't feel like they can keep running over to me. This is very disruptive for instructors who need to know where children in their care are at all times. If my child does run to me because I have remained onsite, Adventure Pirate staff will assume that my child is no longer in their care and will not be included in the remainder of the session.
Adventure Pirate are all inclusive, accept that most home eduacted children have specific needs and that every child is different. I accept this too. I will not speak negatively about children that attend HomeEd sessions to other parents and will not take it upon myselft to deal directly with any incidents that may have occured during a session. I will report any incidents/complaints to Adventrue Pirate staff.
