I have the authority to and do give permission for this child to participate in the Miss Oklahoma Elementary, Junior High, High School & Collegiate America Pageant and have her photo taken and released. The Miss Oklahoma High School America Organization does not issue a refund if you are no long able to attend.

I further release the Miss High School America Pageant Organization and affiliates of any and all liability arising out of my child’s participation in the Miss Oklahoma Elementary, Junior High, High School & Collegiate America Pageant.I release owner, director, staff members, judges, associates and favilities from any and/or all liabilities that may occur due to entering this pageant, such as theft, loss, accident, and/or injuries. I understand that I will be held financially responsible for any destruction of property that our party may cause willingly or otherwise, and that by singing this form, I am relinquishing all rights to file any claims pertaining to, but not limited to, the above. I understand that my/my child's image may be used for marketing purposes by Director.
Fees:

City Title is $225. This is your deposit to the pageant. All contestants are required to obtain a city title. Complete the City Title Request Form and send payment at time of submission.

Elementary: $325

JH, HS, Collegiate:$425

Pageant fee balance due no later than Aug 14, 2025

(Fees do not include hotel accommodations, hair/makeup styling, photos at pageant, travel expenses while at the state pageant)
