INSTRUCTIONS FOR PAYING BY ZELLE
1. Complete the form and hit the submit button (after reviewing these instructions).
2. Go to your Zelle account and add treasurer@lavistapark.org to your Zelle recipients.
3. Send payment amount of $225 via Zelle to the LVP recipient above (treasurer@lavistapark.org).
4. When entering the payment, please specify your LaVista Park property address in the Memo box of the Zelle payment. Failure to provide property address will delay your membership onboarding.
5. You will receive a Thank you pop up after hitting the submit button. The thank you message will contain the LVPCA Zelle ID (treasurer@lavistapark.org). An email with a summary of the jotform submission from the LaVista Park Civic Association will be sent to the email address specified on the jotform, and an email from your bank summarizing the Zelle transaction.
6. The sign up process is not complete until both the jotform are submitted, and the payment via Zelle has been received.
