Mentee Agreement

Please read the following statement and sign this agreement below before submitting. (Parent/Gaurdian is to sign for 16 and under)

I acknowledge that by applying to serve as a mentor I am committing to the following:

1. Being matched with the selected mentor in this engagement for a period of 1 year from the commencement of the agreement.

2. To contact my mentoring partner at least once a month via any mode of communication, along with an in-person meeting minimum six to nine times over the engagement.

3. Responsible for arranging the logistical details of the meetings such as meeting agendas, record sheet etc. (17 or older)

4. Identifying the goals that I wish to work on and being open to my mentor’s advice and feedback to make progress toward my stated goals.

5. Understanding that confidentiality is important in a mentoring relationship, and I will work with my mentee to establish our common expectations of confidentiality. (17 or older)

6. Attempt to address any issues with my mentor directly first and notify a member of The Odis Ella team if we are unable to resolve the issue in case when the relationship becomes unproductive or compromised.

I consent for the team of The Odis Ella Foundation, on behalf of the above forementioned mentee, to hold the data contained in this application form, along with any resume and bio information, and to use for matching with a mentoring partner,monitoring and review as applicable, as part of the mentoring program.
