As a parent/guardian, I certify that my son/daughter has permission to participate in the 2024 Heather Moore-Martin Volleyball Camp. All medical bills, regardless of their nature, whether sickness or injury, shall be the participant's responsibility and not the Heather Moore-Martin Volleyball camp or Pima Community College. Should a health emergency arise, I will be notified, but if I cannot be reached, such medical treatment deemed necessary will be administered by competent personnel.
