This online request form must be filled out completely prior to booking at The Arch, Cosmetic & Fine Line Tattoo Studio. For new & existing permanent make up clients, this form will be available for use. Please allow some time for me to respond as it can take a bit of time to filter through submissions. I typically book in 2-3 month increments at a time. I’ve been working so hard behind the scenes to roll out this new system for you all. This business is a true labor of love & I look forward to sharing it with you all!

Photos should be taken with no makeup on the brow/forehead area and should not be cropped. Photos should be taken in natural light (in front of a window or in the car is best). Current clients please also submit if requesting a touch up to make sure adequate fading has occurred.

Booking Fee- There is a $50 booking fee for ALL services, that is required to be paid when you schedule your appointment with Ashley. This is non-refundable. This fee will be deducted from your total price after completion of service. The $50 booking fee may be applied & deducted when cancelling with less than 48 hours’ notice. Rescheduled appointments can transfer their booking fee but will still need to pay the cancellation policy, if necessary.

Cancellation Policy- There is a 48 hour-50% cancellation policy at The Arch. This means that if you cancel without giving at least 48 hours notice or no show, there will be a 50% cancellation fee of the quoted price that you must pay in order to reschedule. Rescheduled appointments are at full service price. (A full policy document & waiver will be sent via email when you schedule an appointment and can be found on website)

Card on File- A card is required to be held on file when making an appointment at The Arch. This card WILL NOT BE CHARGED when you make an appointment, it is simply saved on file. Payment will be processed at the appointment only once the service has been performed. Cash, Card & Venmo are all accepted forms of payment during booked private appointments.
