  • The ETIP Application Requires The Following: 

    1. Student's School Picture or Headshot.

    2. Student's Most Recent Report Card or Transcript.

    3. Student's Updated Vaccination Record.

    4. Student's Comprehensive or Annual School Behavioral Report OR the Name, Email & Phone Information for a Member of School Leadership Who May Vouch for Your Student Upon Request (no letter of recommendation required). 

    5. Family Emergency Contact(s) Name, Email & Phone.

    6. Family's Pediatrician Office Name, Phone & Address.

    7. Please be advised, while the ETIP application is FREE, ETIP tuition fee is $300.00/weekly with the largest tuition discounts, scholarships and financial aid offered to families with multiple children enrolled, who enroll for multiple-week enrollment and/or for ETIP alumni families. If you have recieved a tuition discount code, please enter it in the box below. For questions regarding discounts, financial aid and scholarships, please email us at etip@pqc.edu

  • General Publicity Waiver

    With the realization that publicity of programs helps ensure the success of the programs, I hereby grant the Early Talent Identification Program at Paul Quinn College full permission to use my/our full name, personal success story, photographs, films, and video of me/my family for the purpose of publicizing the Early Talent Identification Program. This permission includes but is not limited to identification of my child by name in printed materials, reports, grants, website, Facebook, and other social media.

    I therefore grant permission to Paul Quinn College, its employees, or agents to take photographs, videotape, or digital recordings of me/my child and to use these in any and all media, now or hereafter known, and exclusively for the purpose of Early Talent Identification Program (ETIP). I consent that my/my child’s school photo, name, grade and school name may be revealed with or by descriptive text or commentary on the Paul Quinn College and Early Talent Identification Program website.

    I release to Paul Quinn College, its employees, staff, trustees, agents, students, volunteers associated with the event or activity, or officials with participating organizations and sponsors all rights to exhibit this work in print and electronic form publicly or privately and to market and or to distribute copies. I waive any rights, claims, or interest I may have to control the use of my/my child’s identity or likeness in whatever media used.

    I understand that there will be no financial or other payment for recording me/my child, either for initial or subsequent transmission or playback. I represent that I am at least 18 years of age or the legal parent or guardian of the participant named above, have read and understand the foregoing statement, and am competent to execute this agreement.

    Paul Quinn College reserves the right to reconsider or rescend any child's enrollment (named on this application), if pertinent information regarding medical, psychological, educational, or other relevant evaluation or medical care has not been adequately shared with the ETIP administration. If we are unable to accommodate your child’s/children's level of function or if their behavior limits their ability to participate in or benefit from our program, we reserve the right to reconsider your child’s attendance.

    Paul Quinn College reserves the right to reconsider or rescend any child's enrollment (named on this application), if enrollment and tuition fees are not paid in a timely manner.

  • General Medical Waiver

    I give my permission for my child/children to participate in all academic and athletic programming during Paul Quinn College’s Early Talent Identification Program. I understand that in the event of an injury or other emergency my child may be transported across campus by use of a golf cart driven by ETIP or PQC staff.

    In the event of an emergency which affects the health of the participant I, undersigned, do hereby authorize officials of the Early Talent Identification Program at Paul Quinn College to contact directly the persons named on the Emergency Contact Information section of this application and if neither parent nor guardian can be contacted, I authorize the named hospital, physician named and/or the closest hospital selected by the emergency medical care team, to render such treatment as may be deemed necessary in an emergency, for the health of the child. I hereby give the personnel of the Early Talent Identification Program at Paul Quinn College permission to make arrangements for emergency medical attention for all children named in this application, to transport the student(s) to an accredited facility for diagnosis and treatment and to authorize emergency administration of medication as necessary.

    I request and authorize physicians, dentists, and staff of the accredited medical facility to perform any diagnostic procedures, treatment procedures, x-ray treatments and administration of anesthetics as may be necessary in the diagnosis and treatment of all minor participants named on the application. I understand that I have not been given a guarantee as to the results of examination or treatment. I agree to pay for the services rendered and expenses incurred pursuant to this authorization. Further, I will not hold Paul Quinn College, or any Early Talent Identification Program or their Officers, Directors, Administrators, Teachers, Personnel or Employees financially responsible for the emergency care and/or transportation for said child. The authority granted herein will expire one year from the acknowledged date.

    This is to certify that I, as parent/guardian with legal responsibility for all minor children participants named on this application and I do consent and agree to his/her waiver and release as provided above of all the Releases, and, for myself, my spouse or co-guardian, heirs, assigns, and next of kin, waive, release and agree to indemnify and hold harmless the Releases from any and all liability, other than “gross negligence” as defined by Texas law, associated with my minor child’s involvement or participation in these programs, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES, to the fullest extent permitted by law.

  • Acceptable Technology Use Statement & Disclaimer

     While electronic resources, including the Internet, are invaluable tools in an educational setting, they do have some disadvantages. For example, electronic materials, particularly from the Internet, may contain items that are inaccurate, illegal, pornographic, profane, defamatory, or potentially offensive to some people and utterly inappropriate. The Early Talent Identification Program firmly believes that the valuable information and interaction available with electronic and digital resources far exceeds any disadvantages, especially considering a remote learning environment. However, The Early Talent Identification Program staff cannot completely protect students from accessing inappropriate materials and specifically denies any responsibility for monitoring students from accessing inappropriate, offensive, illegal, or obscene sites. The Early Talent Identification staff fully expect parents to provide electronic (Wi-Fi) and digital monitoring throughout the length of the ETIP Institute for Black History Leadership course.

    Finally, the Early Talent Identification Program shall not be liable for any student (named on this application)'s inappropriate use of electronic communications, resources, violations of copyright restrictions or other laws, user’s mistakes, negligence, or costs incurred by the user.

    "Everything submitted in this application for all minor applications listed is true and correct. I furthermore understand that ETIP tuition is priced at $300.00 per week before scholarships, tuition discounts and/or financial aid is added. I understand that students enrolled in full-time enrollment (a minimum of four out of the five ETIP Program weeks), are provided priority in reference to all tuition discounts. ETIP alumni and ETIP families with siblings enrolled are also provided priority in reference to tuition discounts. Finally, I understand that full tuition must be processed for each minor applicant named on the application, the Friday prior to the start of each ETIP Week, otherwise my student(s) will be dismissed from the Program."
