Custom Party Favor Order Form

Please fill out the whole form to ensure proper customizations. After choosing your theme choose the items you would like to order. Each order comes with 12 items. Please note that pricing includes your full customizations. After submitting your form, you will receive a confirmation email. An invoice will then be sent to the email provided. Once order is paid in full, I will begin processing your order. There are no refunds on custom orders unless your items are damaged at no fault of your own (ex: during shipment).
