This application is for members of the Greater Ohio Bleeding Disorders Foundation. Applicants must have an immediate family member affected with a bleeding disorder living in the same household.

We are excited to celebrate our 70th anniversary as a chapter at this meeting.  Special celebrations are planned!

REFUNDABLE Deposit: If an individual / family is eligible, we will require a refundable deposit during the registration process.  Payment can be made by credit card, check or PayPal, and will be fully refunded at the end of the program. 

The cost is $25/individual application, $50/couple or family.

Travel and Refundable Deposit Reimbursement: Financial assistance for travel and refundable deposit will be made on a case by case basis.  Please contact Tanya: tanya@gobdf.org to discuss options available.
