Thank you for your interest in being a model.

There are a few things that may disqualify you from the Ombré service all together.

PLEASE READ THOROUGHLY:

Restrictions:

•Being under the age of 18

•Pregnant or breastfeeding

•Diabetes or Thyroid Issues

•Acne in or around the brow area

•Keloids easily

•Chemotherapy

•Epilepsy

•Accutane use in the past year

•Heart conditions ex: uncontrollable high blood pressure

•You are on prescribed blood thinner medication

•Use of retinol skincare products area less than 3months before your service

•Use of AHA skincare products less than 2 weeks before your service

•Chemical peels, facial, laser, microdermabrasion less than 2weeks before your appointment

• Alcohol 72 hours before your service

• Coffee 4 days prior to your service

If any of the medical restrictions pertain to you : you will need to obtain and provide medical clearance from your doctor prior to the service.

If any of the skincare applies to you: as long as you plan accordingly you should be fine. For example you stop using AHA skincare for a minimum of 2 weeks before your appointment

Do you consent to the, non-refundable, model appointment deposit?* This field is required.Ombré Brows have a $500-$1000 value but for models it is only $250. There is a $50 deposit to secure your appointment slot and the remainder will be due in cash at your appointment. NOTE: If you fail to appear for your appointment your deposit will be forfeited. NOTE: you will have the opportunity to get your appointment discounted if you appear at your appointment with certain services done : makeup, lashes, hair

•Completion of this survey does not guarantee your selection to be a model

•You MUST wear all black to your appointment: NO lettering on your clothing. No colored headbands, black is acceptable. Hair groomed- no bonnets please. Failure to arrive with the proper clothing will result in your appointment being rescheduled. Alternative garment colors: Grey, Stone, Slate

•Your service time may take longer than 4 hours due to the content requirement

• You will have an opportunity to get your service or touch up discounted if you arrive at your appointment with your hair, nails, lashes done. (The discount breakdown will be in the email if you're chosen.)

•TIFFANY J, theebrowbae, reserves the right to reschedule your model appointment, at any time, due to the needs of the business.
