STEP 1 Explain in detail the circumstances for each semester you failed to meet the minimum requirements of Satisfactory Academic Progress or the circumstances that have caused you to exceed the maximum timeframe threshold of Satisfactory Academic Progress.*

Any relevant factors such as serious personal illness, or illness and/or death of an immediate family member.(Circumstances not considered to be mitigating are: work conflicts, lack of transportation, or a minor illness of a student or family member). If you have already earned a degree and are seeking another, explain why you are seeking the additional degree.

For maximum timeframe (MTF) appeals provide a job search in your field of study. For satisfactory academic progress (SAP) appeals, provide a signed note from a doctor on professional letterhead if mitigating circumstance is related to personal illness OR a copy of the obituary or death certificate if mitigating circumstance is related to the death of an immediate family member.

STEP 4 Upload your Individual Action Plan (IAP). An IAP must be obtained from your Mid Mentor and signed by the student in ink—typed or computer generated signatures are not accepted. *
