MILTON - Tattoo Request Form

Thank you so much for your interest in booking with me! Please take your time to ensure the following questions are answered properly in order to recieve a prompt response.

I tattoo in a variety of styles, however if your piece is extremely outside of my area of expertise, I may refer you to an artist better suited for the piece.

At this time, I am only working in black and grey. I do not accept finger tattoos, and I don't do coverups on existing pieces, but I may be able to rework them.

What are you looking to get tattooed? If you want multiple pieces, please number them and provide details for each tattoo.

Please give as much detail as possible. Ideally this description is detailed enough to give me a clear understanding of your vision.

Please specify the style of tattoo. Note that I will not be taking on any pieces in full colour, however small pops of colour or linework in colour is acceptable.

Approximate size of tattoo (in inches). If you aren't sure, please use everyday objects to refer to sizing, such as an iPhone, an Airpods case, a toonie, a debit card, etc.

Tattoo Placement (Where on your body do you want to put the tattoo?)

Outer thigh, inner wrist, outer forearm, etc.

Photo of the area on your body where you want the tattoo. This is especially important if we need to work around existing tattoos. If you'd rather send a photo of someone else with a tattoo in the exact spot you'd like yours, that works too.

Please attach up to three (3) reference pictures below to help explain your vision.

If you are wanting a font, please browse dafont.com and screenshots of your favourite fonts to attach. Ensure the name of the font is clearly in the screnshot so I can find it myself if need be!

In terms of availability, please let me know which days of the week work best within your schedule. Select all that apply. Please note that weekends are the most popular request, and as a result are mostly booked throughout the summer.

If there are very specific dates or times you are trying to make work, please include them here. Note that it isn't always possible for me to accommodate specific dates, but I do try!
