Your personal information included in this Enrolment form may be used or shared by Level Up Academy for statistical, administrative, regulatory, and research purposes. You may also receive a student survey from a government department or NCVER employee, agent, third-party contractor, or authorized agencies. If you are contacted for the survey, you may opt out at that time. We offer comprehensive support throughout your course, please communicate your needs with Level Up Academy or your designated trainer. If you have any specific needs related to disability, please inform our staff before enrolling.

Student Rights:
You have certain rights as a student, including the right to report harassment, discrimination, or abuse. You may also raise concerns or complaints regarding our services or training procedures. For appeals, please address them to the CEO in writing. You also have the right to access your records.

Unique Student Identifier (USI):
All students are required to provide a USI. If you do not have a USI, please apply for one online and complete this section at the beginning of your enrollment form.

Additional Media Consent
Media Consent and Release:

Throughout the course, personnel or contractors of Level Up Academy may capture audio recordings, photographs, or videos and use samples of student work for promotional purposes. All of these materials will be used solely by Level Up Academy and will not be sold or disseminated to any third party. Some of these media assets may appear in promotional editorials and other marketing collateral, including public and professional publications, social media platforms, and similar mediums. By agreeing to this clause, you acknowledge your consent to participate in such activities. If you want to access or obtain copies of any of these photographs or videos beyond standard distribution, you must submit a written request to the ELICOS Operations Manager. Additionally, we request your permission to receive updates and promotional communications from Level Up Academy via email and newsletters, and to allow Level Up Academy to showcase media, comments, and/or samples of your work.
