*Please note that your parent/guardian's consent is required to participate. Please obtain their permission BEFORE submitting your application. Parents and Senior Models are required to sign a contract upon being accepted into our model program.

I will contact all chosen Senior Model applicants and their parents by email with an invitation to the model team program. At that time you will be asked to choose your Senior Model Package. Please review my Senior Model Info page again so you are familiar with the Senior Model package options and your payment plan options. Applicable sales tax will apply to all session, package and product pricing.
