Should you choose to complete your recommended level of care at Mills Treatment Services, Inc. these are the program requirements and fees:
Program Policy:
1. Client does phone sessions a minimum of once a month.
2. Each client must sign a release of information and confidentiality with their Probation Officer to be sent to MTS, Inc.
3. If a client has not made a payment by month three of entering the program, then sessions will stop until client makes at least a partial payment. Once the client makes a payment, sessions will resume. It does not have to full payment, but a
payment. If this occurs, the time off will not count towards your treatment time.
4. MTS, Inc. will update client’s probation officer after each session of attendance and progress.
5. If client is put on hold for nonpayment, MTS, Inc. will inform client’s Probation Officer.
6. Completion of the SOP program is full payment of treatment and completion of all sessions.
7. A completion letter will be sent to the client’s Probation Officer once client has completed the program.
8. If a client does not answer for their phone sessions, then a $25 charge will be added to their balance.
9. If a client misses two consecutive sessions, then an extra month will be added to their program and an extra payment of $70.
10. If a client misses three consecutive sessions, then the client will have to start the program over and will be updated with new balance.
11. If client has not paid a minimum of half their balance by month four, then sessions will be stopped until payments are caught up and could result in additional charges.
12. It is the client’s responsibility to make payments. If client has others making payments for them, then the client will need to get them to call MTS, Inc. or pay through Cashapp. MTS, Inc. will not call others to go over charges. Client can give others the policy of MTS, Inc. to inform them of fees.
Fees of the Program:
1. Assessment fee=$250; Sessions=$80 per session; Processing=$50
2. Ways of making payments: Cashapp at $MonaMills0721 or by card on the payment link of the website.
