Course of Study Scholarship Request Form
For the Course of Study (COS) or Advanced Course of Study (ACOS) courses, Certified Candidates or Licensed Local Pastors may request assistance of 100% of the cost per course.

To request a scholarship, first acquire a PDF copy of your COS bill or invoice from the school, and then fill out the Course of Study Scholarship Form below.

It is preferred that Course of Study grant/scholarship funds be paid directly to the school or your church. If you have already paid the tuition personally, you may request reimbursement with the receipt provided.

If the tuition has not yet been paid to the school, an invoice for the total amount is required. If tuition has already been paid and needs to be reimbursed to your church or to you, a receipt documenting the payment date and amount is required.
