    If your child was born abroad please also bring in their passport.

  • Parent/Legal Guardian Contact Information

  • Emergency Contact Details

    This is the person/people School will telephone in an emergency i.e. if your child is sick, injured or needs collecting from School.
  • School Meals, Allergies and Medical Information

  • All Children from Reception Class to Year 2 are eligible to receive a universal free school meal as part of the government's initiative.
    Children in Nursery, and Year 3 to Year 6 will be charged accordingly.

  • Allergies and Special Diets

  • Please download, from the School Website, an 'Administration of Medicine Request Form' and return this to School, prior to your child's first day at Gledhow.

    https://www.gledhow.leeds.sch.uk/medical

    Without this form, School cannot administer prescribed medication to your child and you would need to come into school to administer the prescribed medication yourself. For certain medical conditions eg asthma, epilepsy, diabetes etc it will be necessary for school to put together a care plan for your child. Please make School aware, as soon as possible, if you think your child needs a care plan.

  • Ethnicity, Religion, Pupil Premium & Special Educational Needs

  • Pupil Premium is an entitlement that comes initially from the eligibility for free school meals. This is different to the universal free school meals that children in Reception to Year 2 are all automatically entitled to.

    If parents or your child are in receipt of one of the following benefits below then your child may qualify for free school meals and will continue to receive a Free School Meal past Year 2.

    After 1st April 2025 the criteria for free school meals has changed. If you apply for free school meals after this date and your financial circumstances change free school meals will automatically stop.

    Free school meals qualification also gives discounts off school trips and enrichment activities, so it is worth investigating if your child qualifies. To check if you qualify then please visit

    https://www.leeds.gov.uk/schools-and-education/apply-for-free-school-meals

    or visit Gov.UK website for further info.

  • You may be entitled to free school meals if your household has No recourse to public funds (NRPF) if you meet the following criteria;

    • has 1 child and a household income of £22,700 or less, or;
    • has 2 or more children and a household income of £26,300 or less and;
    • has less than £16,000 in capital and savings

    Do you qualify under this criteria? If so, please contact the School Office.

  • Permissions

  • Photographic & Video Permissions

    In order for school to comply with the General Data Protection Regulation and UK Data Protection Act 2018, we are required to obtain your consent so that we may take and use photographs and video recordings of your child.

    Photography and videography may be used at Gledhow Primary School for the purposes set out below. Only images of children in suitable dress will be recorded and shared. Staff are not allowed to take photographs or videos on their personal equipment. When sharing photographs with external third parties we will endeavour to:-

    - avoid providing the child's name where the child's image is shared;
    - avoid providing the child's image where the child's name is shared;
    - use only children's first names, rather than their full names (except in exceptional circumstances where we may provide the first initial of their surname to distinguish them).

    When sharing video recordings with external third parties, we will endeavour to:-
    - avoid providing the children's names within the video recording;
    - avoid providing children's full names in crediting of video recordings.

    If we would like your child's image linked to their name we would contact you separately for permission (for example, if your child had won a competition and wanted to be named in press/literature).

  • We would like to contact your child’s current nursery/childcare setting to gather further information that will help us settle your child and place them into a class. By completing the following questions your are giving us consent to make contact with your child's current nursery/childcare setting.

  • Gledowls Before & Afterschool Club Childcare Provision

  • Gledowls is our 'wrap-around' childcare provision/club and was established in September 2018. It is based onsite either in the school hall, IT suite or Gledowls annex building, which is situated near the car park. Experienced playworkers care for the children with the club being managed and overseen by Miss Knight and Miss Pitter, two of school's highly experienced school staff members. The club provides a safe environment, which is both stimulating and fun, focusing on nurturing your child’s social and emotional health. Safeguarding is at the forefront of everything we do - our club is inclusive and committed to an anti-discriminatory practice and recognises children’s diverse circumstances.

    We offer a Breakfast club with drop off from 7.30am (£5.50 per session) including breakfast and an After School club with pick up until 6pm (£12.50 per session) including a teatime snack. Children are taken to or from class at the beginning and the end of the day, giving parents peace of mind that their child is safe, cared for, in line with all the schools key policies and procedures.

    For further information, please visit our school website:

  • Data Protection

  • Under Data Protection law, individuals have a right to be informed about how the school uses any personal data that we hold about them. We comply with this right by providing 'privacy notices' (sometimes called fair processing notices) to individuals where we are processing their personal data.

    We only collect and use pupils data when the law allows us to. Most commonly, we process it where:
    - We need to comply with a legal obligation.
    - We need it to perform an official task in the public interest.

    We do share data with other schools and our 'Cluster' in order to access support and guidance for pupil and families in our school.

    Where we have obtained consent to use pupils' personal data, this consent can be withdrawn at any time. We will make it clear when we ask for consent and explain how consent can be withdrawn. Some of the reasons listed above for collecting and using pupils' personal data overlap, and there may be several grounds which justify our use of this data.

    Further details of how we collect, store and use personal data about pupils can be found by reading our Privacy Notices on our School website.
