Chapter Interest Form

Please fill out the form below if you have interest in starting a PFB chapter at your fire department. We host 3 meetings per month that explain what it looks like to become a chapter. You will have the option to schedule one on this form or schedule one later. Please follow the directions on the email that you will receive once this form is completed. This must be completed by a member of the fire department or fire district.
