DOG/PUPPY ADOPTION CONTRACT

We do our best at PETPROMISE to find the perfect home for every pet. And since it is hard on a pet to be moved from home to home, it is our goal to find the right home the first time.

Every rescued pet comes with its own unique story and history. We do not know everything that this dog or puppy went through before they came into our care and before you came to their rescue. For this reason we can’t guarantee that they will be the “perfect” pet immediately (or ever.)

All pets take, patience, time, work, and commitment-some require more than others-but in some ways that makes your commitment that much more important and special. Their second chance at life starts with you!

By signing below, you are stating that you understand the mission of PETPROMISE; you understand what is expected of you with respect to caring for this pet; and you are ready, willing and able to make a lifelong commitment to this pet.

You understand that PETPROMISE is placing the above described DOG with you, the ADOPTER, as a companion and family member. And in consideration of the mutual covenants in this Contract and other good and valuable consideration, intending to be legally bound by this Contract, YOU agree as follows: AS THE ADOPTER I AGREE TO: Treat the DOG as a family member and keep him/her as an INDOOR pet.*

I Will.

Provide the DOG with fresh water, wholesome dog food, medical care, and shelter for his/her lifetime.*

I Will.

Take the DOG to a licensed veterinarian of your choice within 14 days of the adoption for a wellness exam.*

I Will

I understand that PETPROMISE has made no guarantee regarding the health, age, breed, or temperament of the DOG.*

I Do.

I will take this DOG to see a veterinarian at least once a year and provide medical care recommended or necessary for the good health of this DOG for his/her lifetime.*

I Will

I will provide a secure fenced yard, with ample room for outdoor exercise for the DOG. If fencing needs repair or modification in order to ensure the safety and well- being of the DOG. I agree to make any necessary repairs and maintain the security of the fencing at all times in order to protect and secure the DOG. I understand that gardeners, contractors, utility workers, etc. are not responsible for the safety of the DOG, therefore I will ensure that the that the DOG is kept indoors when workers have access to the yard. I also understand that many dogs are sensitive to and scared of loud noises, and I agree to keep the DOG indoors or in a safe and secure environment on holidays such as July 4th and New Years Eve.*

I will keep the DOG on a leash and under my control at all times when in public places.

I Will.

I obtain any and all licenses required by the State and/or local authorities with respect to the DOG.*

I Will.

I/we will feed the dog at least twice a day and will provide and have a fresh supply of water available for the DOG at all times.

I promise that the DOG will live inside our home and will not be isolated from the family and that I will never leave, nor let any other person leave the DOG outside, even in a fenced yard, when no one is home.

I Do.

Ensure that the DOG will be properly restrained using a secured crate at all times when transported in any vehicle.*

I Will.

Make an immediate and serious effort to find the DOG if it ever becomes lost, by (a) filing lost reports with the animal control authorities, animal shelters, and local veterinarians and pet stores, (b) post the DOG as lost on PetFBI.org (c) hang lost cat signs and (d) contact PETPROMISE to provide notice that the DOG is lost and for additional support and advice. Furthermore, I understand that if the loss was the result of negligence or failure to comply with the covenants of this Contract, PETPROMISE shall have the right to retain the DOG if/when found.*

I Will and I Do.

I understand that if at any time in the future I cannot continue to provide proper care for the DOG, I will not abandon, give or sell the DOG to another person, company, organization, medical research, pound, or animal shelter. I agree to do one or more of the following: 1. Obtain approval from PETPROMISE to transfer the ownership of the DOG to a friend or relative. I understand that PETPROMISE must approve the new adopter who will be required to sign an agreement similar to this one. 2. Pay to board the DOG for an agreed-upon time, allowing PETPROMISE to actively seek a new home for the DOG and then relinquish the DOG to PETPROMISE when a home is found. Or 3. Return the DOG to PETPROMISE—after giving PETPROMISE at least five days notice prior to returning in order for PETPROMISE to secure boarding.*

I Understand and Agree.

I will not euthanize the DOG except in the case of the DOG’s terminal illness or injury, or old age accompanied by pain and suffering, which is not likely to be relieved with medical treatment, and in any case, only upon recommendation and consultation with a licensed veterinarian.*

I Agree.

I will notify PETPROMISE of all changes of address and/or telephone within 30 days after the change during the lifetime of the DOG via e-mail to: PETS@PETPROMISE.ORG*

I Will.

Provide for care of the DOG in my will or estate if by no other means than specifying in writing that the DOG is to be returned to PETPROMISE in the event of my death.*

I Will.

Will not abuse or neglect this DOG and authorize PETPROMISE, at their sole discretion, to determine whether or not the DOG has been abused or neglected and permit PETPROMISE to follow up on the adoption via telephone, e-mail or scheduled home visitation.*

I Agree.

I understand and agree to abide by the terms and conditions of this Contract and agree that compliance with these terms conditions is in the best interest of the DOG.*

I Do.

I hereby release PETPROMISE from any and all liability for personal injury, property damage, legal fees, or veterinary care, or any other expense or liability incurred by me as a result of the adoption of the DOG and I agree to indemnify PETPROMISE from any and all such claims and to pay, without limitation, any costs related to such injury, damage, or liability, including, in the case of litigation, any attorney fees incurred by PETPROMISE in its defense. This Contract is the entire agreement between you and PETPROMISE and supersedes any prior understandings between you and PETPROMISE with respect to the subject matter of this CONTRACT. No modification of this Contract will be valid unless in writing duly signed by ADOPTER and PETPROMISE. This Contract is binding upon you and your heirs, assigns, successors, personal representatives, and executors. By entering into this Contract, you understand that any failure to uphold the covenants made in this Contract will constitute a breach of contract. In the event of any such breach of contract, you authorize PETPROMISE to reclaim both possession and ownership of the DOG. I certify that I have read and agree to the terms and conditions of this Contract in exchange for my adoption of this DOG.*
