After sending in your form, If your idea is accepted, you'll receive an email or text to discuss the date/time!

You have to be 18+ in order to book or get tattooed in the state of RI, NO EXCEPTIONS.

This form is used in place of a consultation

 
•Designs are NOT sent ahead of time as all designs made are custom.

•Creative freedom is MUST in order to create the best design suited for the size and placement.

•PLEASE TRUST ME, being extremely can prevent the tattoo from reaching it's full potential!

Once your form is accepted, I will require a $100 NON-REFUNDABLE deposit (goes towards your final price) thru Cashapp, Venmo, Zelle, or cash in person.

This is REQUIRED to book. However, I do appreciate you taking the time and wanting work from me!

Meaning no deposit = no appointment.

 

If you don't hear back from me then it means I've unfortauntely declined your tattoo or idea!

Do not take it personal, it may not have been the best fit for me or isn't something I'm looking to do at the moment.
