· I release and waive any claims against the NFLFL (National Future Leaders Flag League) Program their employees, sponsors, agents, and affiliates for any personal injury, property damage, or wrongful death that may arise as a result of the Participant's participation in the Program, caused by the ordinary negligence of the parties listed above, wherever, whenever, or however, the same may occur.
· I acknowledge that those listed above are not responsible for any injury or property damage arising from the Program, even if caused by their ordinary negligence.
· I certify that the Participant is in excellent physical health and may participate in strenuous and hazardous physical activities, including football played in the Program.
· I understand that this agreement is intended to be as broad and inclusive as permitted by the laws of the province in which the Program takes place. If any portion of this Agreement is invalid, the remainder will continue in full legal force and effect.
· As the parent or legal guardian of the Participant, I am of legal age and freely sign this Agreement. I have read and understood this form, and I acknowledge that by signing it, I am giving up legal rights and remedies
· I represent that I am the parent or legal guardian of the child named above, and that the terms of this release are binding on the child and me.
· By signing this Agreement, I acknowledge that I have read and fully understand the contents of this waiver and voluntarily agree to its terms.
