Sparky Appearance Request Form

Sparky is for Fire fans of all ages! This Pup is perfect for community & private events in the Greater Chicago area, from grand openings, parades, fundraisers, school visits, birthday parties, and so much more. Please complete the following form to request an appearance from Sparky for your event.
Please consider the following when requesting an appearance:
- Completion of this form is only a request and does not guarantee an appearance
- The maximum length of an appearance is 1 hour. (Parades are exceptions)
- Payment must be received no later than 48 hours before the scheduled appearance.
- An additional $1 per mile will be added for appearances 25+ miles from our Offices at 1 N Dearborn St

Terms & Conditions

Mascot Appearance Guidelines

• On-site Contact must be available by phone up to 45 minutes prior to the requested appearance. We rely on this person to find private room and review event details.
• Sparky can only participate in activities that do not involve speaking.
• Sparky cannot participate in activities that may cause bodily harm or injury to the performer.
• It must be understood that the performer's maximum time in the costume without a proper break is 45 minutes - 1 hour and less than that in excessive heat and humidity.
• The appearance fee must be paid in full no later than 2 (two) business days prior
to the scheduled event.

How to arrange an appearance
• A completed mascot request form must be submitted at least 1 (one) week prior
to the event.
• Upon receipt of the request, the Mascot Specialist will review and contact the requestor by phone or email to confirm or deny availability.

• Football home/away, men's basketball and women's basketball game days take priority over all requests. No appearance can coincide with a game or pre-game activities.
• If Sparky is available, the Mascot Specialist will finalize logistics, and a an invoice for the agreed-upon appearance fee will be sent.
• Confirmation for the appearance will be made after payment has been received
for the appropriate amount.
• Please note that completing the mascot request form does not guarantee a Sparky appearance.

• A Sparky appearance at an event does not constitute endorsement or advertisement of any product, service, view or person. If you are interested in promoting or advertising Sparky's precense at your appearance upon approval, please directly contact our Mascot Specialist here: ddavila@chicagofirefc.com

• Completion and submission of this appearance request form constitutes an agreement to the guidelines listed above, even if submitted electronically without a signature

Cancellation
• In the event that the scheduled appearance is canceled, the Chicago Fire will
grant a refund of the appearance fee on the following basis: a cancellation made at
least five (5) business days prior to the event--full refund; cancellation made five
(5) to two (2) business days prior to the event--50% refund; cancellation later
then two (2) business days prior to the event--no refund.
• If there is a change in the event date for any reason, please notify the Mascot Specialist as soon as possible. The Chicago Fire will do their best to accommodate availability.
• The Chicago Fire may reschedule or cancel an appearance for any reason,
including changes in the Match schedule and unsafe or inclement weather.
In the event the Chicago Fire must cancel an appearance, you will receive a full refund of the appearance fee.
• The performer reserves the right to walk away from any appearance they deem
unsafe, harmful, or uncomfortable with no refund provided.
