EFL Spring-Summer Soccer League Participation Consent and Acknowledgement
I hereby give my consent to participate in all activities associated with the Evolution Futbol League (EFL) Spring-Summer Soccer League. By agreeing to take part, I acknowledge and accept the risks associated with these activities. I release, absolve, and hold harmless the EFL, including all its officers, agents, and representatives, from any liability for injuries that I may sustain during the league's duration, including travel to and from the events.
In the event of an injury to myself, I waive all claims against the Evolution Futbol League, its coaches, affiliates, participants, sponsors, and, if applicable, the proprietors of venues used for the league events. I understand that there is an inherent risk of injury in sports activities, including soccer, which may range from minor injuries to severe conditions such as fractures, paralysis, or even death.
