  • Just like in a traditional hair loss consultation, we would like to ask you some questions about your medical history and any scalp and hair symptoms. Your responses will be completely confidential and will help Dr Ball form a diagnosis and advise on your suitability for the treatment requested.

  • Have you already attended a consultation with The Maitland Clinic (remote or in person)?*

  • Our hair loss medication services are currently only available to patients who have already attended an in-person or remote consultation at The Maitland Clinic. 

    If this is not the case, please contact us on reception@themaitlandclinic.com or 02392 706122 to arrange an appointment before proceeding with your medication order.

  • Are you experiencing any scalp symptoms (such as itching, soreness, burning, altered sensations)?*

  • Do you have a family history of hair loss?*
  • Are you currently taking any medication for your hair loss?*
  • Have you had any other treatment for your hair loss (including hair transplant)?*

  • Please submit the following confidential photographs.

    These photographs are confidential and will be kept in your medical record to assist Dr Ball in diagnosing the cause of your hair loss. They can be completely anonymous if you prefer. They will also allow Dr Ball to assess your response to treatment over time.
