About your Innovation
The below application has the following 6 sections:

1. Opportunity Summary (max 250 words)
- What is the problem you are trying to solve?
- How big is the problem, and who does it affect?
- What is currently being done about the problem, and why isn't it working? Provide some context.
2. Solution Summary (max 250 words)
- Proposed solution: Describe your idea in a brief, succinct, and clear statement. (Max 1-2 sentences)
- Impact: Describe the overall impact that you hope to achieve and how you will measure it.
3. Stakeholders and Resources (max 250 words)
- Who will you need to present your solution to to get support for and begin implementing your solution?
- What resources is your institution providing to support your solution? (Examples: effort, funding, supplies, etc.)
- Teams must have at least one member of executive leadership as an advisor or sponsor to implement innovations within the health system (Examples: Chief Nursing Officer, Chief Executive Officer, Chief Finance Officer)
- What support do you anticipate you will need? What barriers will you anticipate?
- *If selected as a finalist you will be required to submit a letter of support from executive leadership.
