  • About your Innovation

  • The below application has the following 6 sections:

    1. Opportunity Summary (max 250 words)

    • What is the problem you are trying to solve?
    • How big is the problem, and who does it affect?
    • What is currently being done about the problem, and why isn't it working? Provide some context.

    2. Solution Summary (max 250 words)

    • Proposed solution: Describe your idea in a brief, succinct, and clear statement. (Max 1-2 sentences)
    • Impact: Describe the overall impact that you hope to achieve and how you will measure it. 

    3. Stakeholders and Resources (max 250 words)

    • Who will you need to present your solution to to get support for and begin implementing your solution? 
    • What resources is your institution providing to support your solution? (Examples: effort, funding, supplies, etc.)
    • Teams must have at least one member of executive leadership as an advisor or sponsor to implement innovations within the health system (Examples: Chief Nursing Officer, Chief Executive Officer, Chief Finance Officer)
    • What support do you anticipate you will need? What barriers will you anticipate?
    • *If selected as a finalist you will be required to submit a letter of support from executive leadership.

    4. Budget (Please enter the amount requested. Only ask for the amount you need.)

    5. Team Members (Please upload a document with your team members and their credentials, titles, and emails. Please note: Teams should be interdisciplinary.)

    6. Upload Pitch Slide Deck. (Please note: You must use the template provided for your pitch.)
