Consent* This field is required.As the parent/guardian (identified in Box 1B) of the child (identified in Box 1A) I hereby provide permanent consent for my child’s work submitted with this release - including videos, photographic images, drawings, paintings, mixed media, handwritten or typed short essays, poetry and/or other creative written piece of work (Materials) - to be posted by the Ontario Bar Association (OBA) on its website and social media sites (including Twitter, Facebook Instagram, and LinkedIn). I further consent to have my child’s first name used by the Ontario Bar Association on its website and social media sites, either separately or in association with his/her Materials or any voice recording, video recording(s), audio recording(s), soundtrack(s) and photograph(s) made of my child (Likeness) at Law Day events. I warrant and represent that the Materials: (i) are original to my child, and that (ii) no additional rights or consents are necessary to grant the rights to the OBA as provided herein. I acknowledge that the OBA’s decision to use the Materials, and my child’s Likeness as set out herein is at the OBA’s sole and absolute discretion; I understand and agree that neither my child nor I will have any right of approval and will not receive any compensation for his/her participation in this competition or for the use, by the OBA on its website or social media sites, of his/her Materials, or Likeness.
