Puppy Application- Standard Poodle

Thank you for your interest in adopting a puppy from Lady Mae's Standard Poodles! The following application makes it possible for us to get to know our potential fmailies in depth to ensure that each of our puppies are placed with the perfect family to fit their lifestyle!

Please read all of the following information carefully and complete each section of the form!

All of our adult dogs are AKC Registered and are being genetically tested through Embark. All of our upcoming females will be OFA certified. We offer our standard poodles with limited AKC Registration. Full AKC Registration (breeding rights) may be discussed and offered to select breeding programs at our discretion!

All of our puppies are raised inside our home and receive daily interaction from us and our other pets.

Our puppies are dewormed at 2, 4, 6 and 8 weeks of age. They will recived their first round of vaccines and be seen by our vet at 6 weeks of age. Every puppy will go home with a care package to assist in the transition into their new enviornment!

Puppy selections are made around 6 weeks of age in the order of our waitlist deposits. After the puppies recieve their firstr round of vaccines, we will schedule a "pick day" fir those who choose to visit us and select their puppy in person. For families that are long distance or choose not to visit, we offer a puppy "pick day" via FaceTime or through picture snad vidoes.

Finally, we welcome our fmailes to pick up their new baby at 8 weeks of age. We will schedule a "gotcha day" accordingly! We do offer delivery to those who need or prefer that option, for an additional fee!

Our adoption fee is $750. A non-refunadble depost of $250 is required to reserve a puppy on our waitlist. This includes a $100 fee to be added to our official waitlist andf then the remaining $150 is due within 48 hours of puppies arriving. *deposits are not accepted until applciations have been reviewed and approved. We accept 6 early deposits per waitlist- 3 male, 3 female. In the event that we have a small litter or that there are not 3 puppies of each gender, we will give out early deposit families the choice of the opposite gender or totransfer their deposit to the next waitlist of their choice! We do ask taht families please inform us if their circumstances have changed or they have adopted a puppy in the meantime of joining our waitlist. This opens up the opportunity for families that are looking to join our waitlist.

Once an applicartion is recieved, we will contact families with more information about our upcoming litters and waitlists. When an an application is approved, we will accept a deposit if there is open availability on one of our waitlists. Please feel free to contact us for further information or questions.
