This form is for individuals who wish to surrender their cat or a found cat to North Beach PAWS, located just outside Ocean Shores, WA.

Before submitting this form, please provide accurate information to help us process your request. Be aware that the wait time for processing may take one to two months, depending on the shelter's capacity.

You must live within Grays Harbor County.

Our goal is to find a forever home for each cat. However, submitting this form does not guarantee acceptance of the cat or kittens; each submission will be evaluated individually by our staff. North Beach Paws always suggests a friend or family member who will take your family's feline before surrendering to any shelter.

The person submitting the request is responsible for transporting the cat or kittens to our shelter for surrender.

The owner or current caregiver is responsible for their transport.

From time to time we have cats that we cannot rehome. The cats may be returned to you or the area where they are most familiar after spay or neuter and updated shots.*

Cats will not be accepted in a live trap. Cats must be delivered to the shelter in a hard-sided pet carrier. A pet carrier may be borrowed from the shelter upon request. NB PAWS will not trap.*
IF You stated NO on the above question and you are moving out of the area and the cat cannot be adopted because it cannot adjust to the shelter or is aggressive, the cat will be returned to the following person: Name, number, and address.

Authorization and Agreement:

Authorization to Surrender: I, the undersigned, hereby authorize the transfer of ownership of the cat named above to North Beach PAWS.

Right to Deny Application: North Beach PAWS reserves the right to reject this application if any response is discovered to be false or intentionally misleading.

Release of Information: I acknowledge that the information provided herein may be utilized by North Beach PAWS to facilitate the rehoming of the cat and might be disclosed to prospective adopters.

Health Records Release: I consent to the disclosure of all available health records and veterinary details pertaining to the cat to North Beach PAWS.
