The Client recognizes that the law firm Muller Brazil has agreed, at Client's request, to accept this employment as the Client's attorneys in this specific claim; without payment of any retainer fee and without payment of an hourly fee by Client for the investigation and legal work that Muller Brazil will perform. Pursuant to the Vaccine Injury Compensation Program (“VICP”), Muller Brazil will obtain any and all medical records necessary for evaluation and filing of a claim in the U.S. Court of Federal Claims, and file such a claim if deemed appropriate by Muller Brazil. All work performed by Muller Brazil will be compensated directly by the Program, and client will not have any obligation to pay such attorney fees directly.

All costs and fees are paid by the Court of Federal Claims at the end of the case should we file a petition for compensation on your behalf. Any compensation awarded to you will be disbursed to you in its entirety. No attorneys' fees or costs will be deducted from your award pursuant to the Rules of the Vaccine Act. Muller Brazil reserves the right to associate with cooperating counsel. Client understands that attorney fees will be paid to Muller Brazil by the Vaccine Injury Compensation Program. Client authorizes Muller Brazil to endorse, on behalf of client, any lien and any check issued for payment of attorneys’ fees and expenses. 

Should no money be recovered by suit or settlement, said attorneys are to have no claim of any kind against me/us for any professional services rendered. 

It is further expressly agreed that no settlement will be made without Client's consent, that the attorney has exclusive right to take all legal steps to enforce Claimant's claim, and the Claimant will not settle this claim in any manner whatsoever without the prior consent of Muller Brazil. 

Muller Brazil reserves the right at any time to withdraw as counsel for any reason. It is recognized that further investigation of the claim may subsequently change Muller Brazil's opinion concerning the claim, its merit, and the feasibility of proceeding further. Muller Brazil reserves the sole discretion to determine whether to appeal any decision or judgment rendered in this claim. In the event of such withdrawal prior to settlement or judgment, Muller Brazil shall be entitled to such fees and expenses awarded by the Court, and Client agrees to protect Muller Brazil’s right to petition for such fees. 

The Client hereby acknowledges that a copy of this document has been given to the Client and that the Client has been advised to retain it in the Client's possession, and that the Client has read and understood the contents of this document before the Client has signed it.

Consumer Disclosure

From time to time, My Vaccine Lawyer (Muller Brazil) may be required by law to provide to you certain written notices or disclosures. Described here are the terms and conditions for providing to you such notices and disclosures electronically through the JotForm electronic signing system. Please read the information below carefully and thoroughly, and if you can access this information electronically to your satisfaction and agree to these terms and conditions, please confirm your agreement by clicking the ‘I agree’ button at the bottom of this document.

Withholding or Withdrawing Consent

If you decide to withhold your consent to do business electronically you must inform us of your decision to receive future notices and disclosure in paper format. If you consent, you may at any time change your mind and tell us that thereafter you want to receive required notices and disclosures only in paper format. How you must inform us of your decision to receive future notices and disclosure in paper format and withdraw your consent to receive notices and disclosures electronically is described in the “Getting Paper Copies” section below.

Getting Paper Copies

At any time, you may request from us a paper copy of any record provided or made available electronically to you by us. You will have the ability to download and print documents we send to you through the JotForm system during and after the signing session. You may request delivery of such paper copies from us by emailing us at: info@myvaccinelawyer.com. In the body of such a request you must state your email address, full name, address, and telephone number.

Consequences of Opting Out of Electronic Business

If you elect to receive required notices and disclosures only in paper format, it will slow the speed at which we can complete certain steps in transactions with you and delivering services to you because we will need first to send the required notices or disclosures to you in paper format, and then wait until we receive back from you your acknowledgment of your receipt of such paper notices or disclosures.

Electronic Delivery

Unless you tell us otherwise in accordance with the procedures described herein, we will provide electronically to you through the JotForm system all required notices, disclosures, authorizations, acknowledgements, and other documents that are required to be provided or made available to you during the course of our relationship with you. To reduce the chance of you inadvertently not receiving any notice or disclosure, we prefer to provide all of the required notices and disclosures to you by the same method and the same address that you have given us.

By signing above, you confirm that I can access and read this Electronic CONSENT TO ELECTRONIC RECEIPT OF ELECTRONIC CONSUMER DISCLOSURES document; and I can print on paper the the disclosure or save or send the disclosure to a place where I can print it, for future and access; and Until or unless I notify My Vaccine Lawyer (Muller Brazil), I consent to receive through electronic means all notices, disclosures, authorizations, acknowledgements, and other documents that are required to be provided or made available to me by My Vaccine Lawyer (Muller Brazil) during the course of my relationship with My Vaccine Lawyer (Muller Brazil).
