Cat Spay and Neuter Consent Form

Low-income vouchers are issued at the discretion of the spay and neuter coordinator. Residents within the North Beach service area may experience shorter wait times.

If you have multiple cats, please note that a separate form is required for each one. Kittens must be at least three months old and typically weigh around three pounds to qualify for spay or neuter services. Requests can be made earlier, but the procedures will not be performed until the kittens reach the appropriate size and age.

Spay and neuter surgeries will be performed at off-site veterinary clinics, with scheduling managed by North Beach PAWS. Please read the entire form before submitting it.

There is a limit of 2 vouchers per household per calender year.

There is a fee of $60.00 per cat for spay and neuter services. The fee will be collected at the time you schedule your appointment with North Beach PAWS. If you are issued a voucher, the fee must be paid before the voucher is issued. A coordinator will contact you via text regarding the deposit. Cats and kittens are administered parasite protection, which lasts a month, and receive FVRCP and Rabies vaccinations during surgery if they are not already up to date. Do you consent to this?*

The cats will undergo a veterinary assessment, during which they will be spayed or neutered and vaccinated against FVRCP and rabies. Feral cats, or those without a discernible owner, may receive an eartip. This eartip indicates that if the cat is trapped again in the future, it has already been altered and can be released without enduring the stress of another veterinary visit.

North Beach PAWS, its volunteers, the veterinarians, and other caregivers will not be held liable if a cat cannot be saved due to injury, illness, or an undiagnosed medical condition, whether before, during, or after any surgical procedure, vaccination, or treatment.

North Beach Paws does not conduct spay or neuter surgeries. Our service is limited to providing transportation to the clinic where the surgeries are performed.

As the owner or caregiver of the cat, you are responsible for transporting your cat or kitten to our shelter. Cats and kittens must be in a pet-safe carrier. We will not accept cats brought in by trap.

For the spaying or neutering of multiple cats, we suggest a donation to help cover the costs of vaccinations and surgery.

Please note that only one cat per surgery day is permitted.

North Beach PAWS has the right to deny this application if any answer is found to be untruthful or purposefully misleading.

Once your request form is reviewed, a member of our cat spay and neuter team will contact you. Currently, there is a very long waiting list.

If it is kitten season, availability for spay and neuter appointments is limited. Anticipate a wait of a month or more before receiving a text about an available slot for your cat or kitten.
