• Cat Spay or Neuter Request Form

    If you have multiple cats, please note that a separate form is required for each cat submission.

    Kittens must be at least three months old and typically weigh around three pounds to qualify for spay or neuter services.  Procedures will not be performed until the kittens reach the appropriate size and age. You must reapply when the kitten is 3 months old.

    Spay and neuter surgeries will be performed at off-site veterinary clinics that partner exclusively with us, with scheduling managed by North Beach PAWS. 

    Our team reviews each form to see if you qualify for a Low-income voucher. Vouchers are issued at the discretion of the spay and neuter coordinator. 

    There is a limit of 2 vouchers per household per calendar year.

    Form must be submitted online.

    The wait time for a response could be up to a month, depending on demand. Please check your text messages for confirmation.

  • There is a non-refundable deposit of $40.00 per cat for spay and neuter services. The non-refundable deposit will be collected at the time you schedule your appointment with North Beach PAWS. If you are issued a voucher, the fee must be paid before the voucher is issued. A coordinator will contact you via text regarding the deposit. This non-refundable deposit will be collected prior to scheduling, and a voucher will be issued by North Beach PAWS. The coordinator will contact you via text regarding that deposit.*

  • If applying to have surgery for your kitten they must be 3 months old and 3 pounds. Applications will not be processed until that time. You will need to reapply then.*

  • Is Your Pet Current On Vaccinations?*
  • My pet's vaccines were administered last by and when:*

  • Cats and kittens are administered parasite protection, which lasts a month, and receive FVRCP and Rabies vaccinations during surgery if they are not already up to date. Do you consent to this?*
  • The cats will undergo a veterinary assessment, during which they will be spayed or neutered and vaccinated against FVRCP and rabies.

    North Beach PAWS, its volunteers, the veterinarians, and other caregivers will not be held liable if a cat cannot be saved due to injury, illness, or an undiagnosed medical condition, whether before, during, or after any surgical procedure, vaccination, or treatment.

  • As the owner or caregiver of the cat, you are responsible for transporting your cat or kitten. Cats and kittens must be in a pet-safe carrier. We will not accept cats brought in by trap.  Feral cats can also be refused.  Other options can be recommended in that case.  

     

  • North Beach PAWS has the right to deny this application if any answer is found to be untruthful or purposefully misleading

     

  • North Beach PAWS does not conduct spay or neuter surgeries on site.
