Parent/Legal Guardian Waiver and Release of Liability
I, as the parent or legal guardian of the registered summer camper/child, hereby acknowledge that my child's participation in The WAY Center's Summer camp organized and operated by The WAY Center, involves certain inherent risks, dangers, and hazards, including, but not limited to, physical injury, illness, property damage, and even death. I understand and accept that these risks cannot be eliminated entirely, despite the reasonable efforts of The Way Center to minimize them.
In consideration of allowing my child to participate in the activities offered by The WAY Center, I hereby agree to assume all risks and responsibilities associated with my child's participation. These activities are to include on-site and off-site, such as field trips. I agree to release, indemnify, and hold harmless The Way Center, Harmony Church, its officers, directors, employees, agents, volunteers, and representatives from any and all claims, liabilities, damages, losses, or expenses, including attorney's fees, arising out of or in any way connected with my child's participation in the summer camp.
I understand that this waiver and release of liability applies to all activities, whether organized by The WAY Center or conducted independently by my child during the duration of the summer camp. I further acknowledge that I am solely responsible for my child's health, safety, and well-being during their participation in the summer camp.
I certify that my child is physically fit and capable of participating in the activities offered by The WAY Center and that I have not been advised otherwise by a qualified medical professional.
I hereby grant The WAY Center the right to use my child's name, likeness, voice, and/or image in connection with any promotional materials or publications related to the summer camp, without compensation or further authorization.
I have carefully read and understand the terms of this waiver and release of liability, and I voluntarily agree to be bound by its provisions on behalf of myself and my child.
