DATE: June 13th, 2024             TIME: 5PM - 11PM

SET - UP: 4PM 

Southwest Community Watch is a nonprofit organization all registration fees are used to benefit the good of the community that we serve. 

ADDITIONAL INFORMATION:

• Registration fee non-refundable 
• Teams are permitted to begin setting up at 4PM
• Teams are responsible for bringing your own boards and bags (no equipment will be provided)
• Teams are responsible for notifying substitute player if s/he is needed to replace an original team player
• Southwest Community Watch is not responsible for any loss or stolen equipment, cellphones, money or anything else that is at risk of being loss or stolen
• All players take part in cleaning tournament area - clean up begins after tournament is over
• Participants will be asked to leave if s/he causes a disturbance 
• One winning team - Winning Team (both players) will receive a cornhole tournament belt and bragging rights 

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION
OF RISKS 

Please read this information carefully and be aware that in signing up and participating in this program/activity, you will be expressively assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you might sustain as a result of participating in any and all activities connected with this event / tournament.

I recognize and acknowledge that there are certain risks of physical injury to participants in this activity, and I voluntarily agree to assume the full risk of any injuries, damages, or loss, regardless of the injuries, damages, or loss that I may sustain as a result of participating in any and all activities connected with or associated with this event / tournament. I further agree to waive and relinquish all claims I may have as a result of participating in this activity hosted by Southwest Community Watch. I release thier officials, agents, affiliates, volunteers, employees, and sponsors.

I do hereby fully release and forever discharge Southwest Community Watch, including their officials, agents, affiliates, volunteers, employees, and sponsors and all claims for injuries, damages or loss that  I may have and arising out of, connected with, or in any way associated with this tournament / event. 

Entered name below has acknowledged providing the above information to partner and substitute player. They both have agreed to the terms and conditions associated with this event / tournament.
