I hereby give my approval for my child’s participation in all activities prepared by The Community Effect’s Youth Group Activities. Some activities there could be a risk of being injured that is inherent in any activities. TCE is not liable for injuries or damage incurred.Duty of care has been taken in planning activities to mitigate foreseeable hazards and ensure a safe environment for all participants and bystanders. Generally, most activities carry very little risk of injury. The TCE safety standard committee prepares for the physical and emotional well-being of all participants.

I give permission for TCE to use my child’s photo for media purposes. (newspaper, social media newsletter etc.) (please call 218-270-4772 or email info@communityeffect.org for any questions, regarding permissions or photo release. If any part of this form is preventing your from signing up your Child, please let us know.
