Raise a Concern

This form allows you to advise the NSW Institute of Sport (NSWIS) that you would like to raise a concern about Child Safeguarding. It will be received by the NSWIS Child Safe Officer, then actioned and forwarded to the relevant parties. Your concern will be treated confidentially.
You can choose to remain anonymous but if you do, the NSWIS Child Safe Officer will not be able to follow up with you.
