Complete this form to request or cancel an additional appliance. All additional appliances must be individually requested through separate form submissions (one per form) and approved by Management prior to installation. An email will be sent when each request is approved or denied. Review full Appliance Registration details.
Appliances That Require Registration:
Monthly Fee: View the corresponding fee for owning an additional appliance by referring to the current fiscal year's Fee Schedule.
Ceasing Charges + Use: The applicable monthly charges will continue until the cancellation form is submitted. After submission, the appliance should be reomoved immediately. Any appliances still in use after cancellation will be subject to an unregistered appliance fee.
Move-Outs and Transfers: Vacating households with active appliance registrations will automatically recieve a final charge prorated to the move-out date, unless cancelled prior. If the entire household transfers to a new unit, any active registrations and associated chargers will automatically transfer to the new unit on the move-in date.
