Inconsideration of receiving a tattoo from Dana at Boss tattoos I agree to the following:
The person signed in this document has been fully informed of the inherent risk as associated with getting a tattoo. I fully understand that these risks, known and unknown can lead to injury, including any and all future health risks associated in the future, including, but not limited to infection, scarring, difficulties in detecting, melanoma, and allergic reactions to tattoo pigment, nitro /latex gloves, and or soap, including any and all future health risks associated in the future. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed with the Tattoo application, and I freely accept and expressly assume any and all risks that may arise from tattooing.
To wave and release to the fullest extent permitted by law, each of the artist and the tattoo studio from all liability whatsoever, for any and all claims or causes of action that I. ; My state, errors, executors or assigned may have for personal injury or otherwise, including any direct and/or Consequential damages, which result or rise from the application on my Tattoo, whether caused by the negligence or fault of either the artist or the tattoo studio, or otherwise.
That both the artist and the tattoo studio have given me full opportunity to ask any questions about the application of my tattoo, and all my questions have been answered to my total satisfaction.
The artist and the Tattoo studio have given me instructions on the care of my tattoo, thinning medication, I do not have any other medical or conditions that may interfere with the application or healing of the Tattoo. I am not the recipient of an organ or bone marrow transplant, or, if I am, I have taken the prescribed preventative regimen of antibiotics that is required by doctor in advance of any invasive procedure such as a tattoo or piercing. I am not pregnant or nursing. I do not have mental impairment that may affect my judgement of getting a tattoo.
Variations in colour and design Tattoo I have selected the actual Tattoo when it is applied to my body. I also understand that overtime, the colours and clarity of my tattoo will fade due to unprotected exposure to the sun and naturally occurring dispersion of pigment under the skin.
A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which can be disfiguring and/or Costly, which, in all likelihood will not result in the restoration of my skin to its exact appearance before getting tattooed.
I release all rights to any photographs taken of me and Tattoo, and given consent in advance to their reproduction in print or electronic form.
I agreed to reimburse Tattoo for any attorney fees, and cost incurred in any legal action I bring against the artist or the tattoo studio and the artist or the tattoo studio is the prevailing. I agree that the courts of Alberta in Calgary, Canada she'll have personal jurisdiction and venue over me and she'll have exclusive jurisdiction for the purpose of , litigating any dispute arising out of the related agreement.
Acknowledge that I have been given opportunity to read and understand the information contained in this document.
Declared that I am legal age (18+) and have provided valid proof of age, and I am confident to sign this agreement and is complete and understanding and concurrence with this agreement.
I acknowledge that by signing this document and any further tattooing applied prior to this date is bound under the same contract. This single contract embraces all tattooing sessions that further this date, so please take note of all encompassing aforementioned guidelines to apply to any further sessions.
If a deposit has been given, it shall be held until the last Tattoo appointment upon completion. All cancellations without 48 hours notice, Result in loss of deposit. Also, if artwork has been performed by the artist pertaining to the design on the Tattoo, and the client wishes not to proceed with the deposit is taken as payment for the work rendered.
if any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that pertain Shelby, severed from this contract prior to the application of the Tattoo. The remainder of this contract will then be constructed as though the enforcible portion had never been contained in this document.
