Permission Slip

Light & Truth Released Time Bible Education is an opportunity for Fairfield County school students in 1st —12th grade-depending on school district, Bible classes are offered during the school day with parent consent, are located off school property, and are funded by grants and  the local community. Students are released to leave school, for time given by school district, one day a week. Classes are offered during non-instructional times in the school day. Transportation is provided by Light & Truth and trained volunteers accompany students from the school building to a nearby location for a non-denominational Bible class. Volunteers go through an extensive approval and training process, have background checks, and are committed to ensuring child welfare and safety during program times.

 If your child is interested in participating in Light & Truth Bible Education, please complete and return the parent permission slip online. Program participation may be limited by available classroom space. Class start times and dates will depend on the school district your child is in, they will be listed on the school district page in our website https://LightnTruth.org/SchoolDistrictInfo.  If students are enrolled in Light & Truth, they must attend when present at school. Parent Consent Form

I, the undersigned parent/guardian of the student listed below, give permission for the student listed below to attend Light & Truth Released Time Bible Education. Students are responsible for any school assignments missed during released time classes. I may revoke consent for my student to attend the program at any time, and the program may remove any student from the program for disciplinary concerns. I understand that volunteers and program administrators are trained to report child abuse, neglect, and concerns about self-harm. There may be occasions when class times vary from the approximate times provided above. At times, video or photos may be taken during class or special events, and I give permission to Light & Truth, and the state and national released time Bible education organizations with which Light & Truth is affiliated (School Ministries Ohio and School Ministries, Inc.) to use and publish, without charge, photographs or videos of my student. I ask my student's school to provide a copy of my student's emergency medical card to Light & Truth School Ministries, Fairfield County Ohio, so that information is readily available in case of emergency. I understand this parent consent is valid until revoked by me by removing my child from the program. I affirm that I am the parent or legal guardian of the student listed, and I have legal authority to provide this consent.
