I HEREBY ACKNOWLEDGE THE FOLLOWING:

1). The taking of blood or urine samples as regulated by the anti-doping rules of SCB or BE.

2). The supervisory Club/Bowls Representative may on my behalf in a medical emergency consent to my receipt of medical treatment by competent medical authorities, where it would be contrary to the players interest in the Authorities/Paramedic or Doctors opinion should consent not be given.

3). The taking and making of photographs or videos will only be used in conjunction with formal bowls activities of the Club,County (SCB) or Bowls England (BE).

4). I acknowledge that the Club,County or Bowls England are not responsible for providing supervision for the player excepting formal bowls activities.

5).I agree that the above information entered in this document is true & accurate at the time of completion & that I will inform the relative authorities immediately of any changes.

Entry to the Club Competitions is included in the Membership Fee. Only a Full Member is eligible to enter the Club Competitions. For Pairs Matches except the Drawn Pairs please enter both players names.

No Entries will be accepted after the closing date set for the draw or if you are not a Full Paid Up Member.
